The wording you specify will be engraved on the plaque for the award you sponsor.

If you wish to sponsor multiple awards, please complete the form for each award (a link will be provided on the confirmation page for each award so that you do not have to fill in your information each time). If you wish to sponsor multiple awards in the same category, you can indicate that either in the award/position fields or in the comments if you find that more convenient.
